Our in-home care service is focused on providing care and support to patients with chronic illnesses, like Alzheimer’s and other special conditions. We collaborate with the patient’s healthcare provider/s to develop a customized care plan based on the patient’s needs. Our caregivers will make sure that they adhere to their medications and stay safe within their homes, reducing chances of wandering, wounding, and getting lost due to memory problems brought about by the disease.

After this initial contact, our care coordinator sets up an in-home visit, where a registered nurse conducts a thorough assessment of the patient’s needs. This initial home visit is also an avenue for us to interact with the patient and the family as a whole and answer any questions they may have concerning the services and our agency.
